Q. What providers can I see if I have the dental and/or vision plan(s)?

A. If you are enrolled in SHIP and have elected to purchase the optional dental and/or vision plans, you may access the list of providers who participate with First Ameritas.

From this page, you can choose from several options, including:

Find a dental provider: To do so, enter your Zip Code, select "PPO Nationwide," and click on "Continue." Then, select the distance you are willing to travel and the specialty you prefer. (Note: If you enter the zip code 14850, and then select 10 miles and "general dentist, your search will yield 17 choices.)

Find an eye care provider: Type your zip code into EyeMed Access Plan's  web form; then click on "locate nearest providers." For your convenience, we have listed below the three eye care providers that are located within one mile of the Cornell campus.

          • Empire Vision: 722 S. Meadow St.  273-3300
