Optional Vision Plan: 2008 - 09
You may know already how expensive good eye care and eye wear can be. An optional vision plan is available at a competitive price to those enrolled in the SHIP (students, as well as dependents). This plan allows you to receive vision services from a network of providers, both within Ithaca, and outside the Ithaca area.
Download and print the 2007-08 eye care plan brochure and application (pdf); check back in June for 2008-09. You may also pick up a hard copy at the Office of Student Health Insurance in Collegetown or at Gannett Health Services.
For Fall 2008
Eligibility:
- You must be enrolled in the SHIP to enroll in this vision plan.
- To enroll dependents (spouse, same-sex partner (SSP), children), you and they must be enrolled in the SHIP.
Enrollment deadline:
- Fall enrollment: September 30, 2008
- Late registrants: within 30 days of registration
For Spring 2009
Eligibility:
- Due to plan limitations, only students who are eligible to enroll in the SHIP
at the beginning of the spring semester and who do enroll in the SHIP may enroll in the vision plan.
- This includes students entering Cornell for the first time or returning to Cornell after being away for the fall semester.
- Eligible dependents of these students who also are enrolled in the SHIP are eligible.
- Students who were registered at Cornell for the fall semester of 2008, even those who are enrolled in the SHIP, are not eligible to enroll in the vision plan for spring semester only. You may enroll in the plan at the beginning of the next plan year.
Enrollment deadline:
- New spring semester students only: February 28, 2009
- Late registrants: within 30 days of registration
