Optional Dental Plan for SHIP Members 

Costs are rising for all kinds of health services, including dental care. Cornell University students and dependents who are members of the Student Health Insurance Plan may enroll in an optional dental insurance plan that has been tailored to fit their needs.  

This plan, offered through Ameritas Life Insurance Corp. of New York, provides preventive and basic coverage at a reasonable price and access to a nationwide Participating Provider Organization (PPO) of approximately 7,000 New York dental provider access points, with over 80,000 access points nationwide.

First Ameritas Prescription Discount

SHIP members who enroll in First Ameritas dental or eye care insurance can save on prescription medications – for themselves, their covered dependents, even their pets – through the First Ameritas Rx Discount Program. This prescription discount is offered at no additional cost, and it is not an insurance. SHIP members looking to save money on prescription medications may benefit from this program. ENROLLING IN THE FALL

Eligibility

• Students enrolled in the SHIP are eligible.
• Students dependents enrolled in the SHIP are eligible.
• If you are not enrolled in the SHIP, you are not eligible.

Coverage dates

• August 17, 2012 – August 16, 2013 or
• August 17, 2011 – August 16, 2012

Enrollment deadline

• Fall entering students: September 30th
• Late registrants: within 30 days of registration

Annual rates: Same rates for 2012 – 2013 or 2011 – 2012

• Student: $271
• Spouse/Domestic partner: Additional $289
• One or more children: Additional $439

Application

• Submit an application for enrollment online using your Cornell Net ID and password.
  

SPRING 2012 ENTRANTS ONLY

Eligibility

Due to plan limitations, only students who are eligible to enroll in the Dental Plan at the beginning of the spring semester AND who do enroll in the SHIP may enroll in this plan. 

• This includes students entering Cornell for the first time or returning to Cornell after being away for the fall semester.
  
• Students dependents enrolled in the SHIP are eligible.
  
• If you are not enrolled in the SHIP, you are not eligible.

Effective dates for enrollment

• January 17, 2012 – August 16, 2012

Enrollment deadline

• Spring enrollment deadline: February 28, 2012 
  
• Late registrants: within 30 days of registration
